(10.9) Does the Department of Developmental Services (DDS) have forms in other languages?

DDS has these forms in Spanish, Armenian Western, Dari, Farsi, Hindi, Hmong, Japanese, Cambodian, Korean, Laotian, Portugese, Russian, Chinese, Tagalog, Urdu, Vietnamese and Mien. They are all available on the DDS Lanterman Act Eligibility and Service Appeals page. https://www.dds.ca.gov/general/appeals-complaints-comments/fair-hearings-complaint-process/

DS 1819 – Representative Authorization Form – Use this form if you intend to have someone represent you at the hearing. This form gives DDS permission to share information about your case with an advocate or attorney. 

DS 1820 – Notice of Action – This is the form that the regional center must provide you when they intend to reduce or discontinue a service. Starting January 1, 2025 it will also be used to inform people of intake and eligibility determinations. It also lists contact information for OCRA, the State Council on Developmental Disabilities (SCDD), and Disability Rights California.

DS 1822 – Notice of Resolution – If you have filed an appeal and your issue gets resolved before the judge makes a decision, or if you wish to withdraw your appeal, then this form is used to explain how the case was resolved. 

DS 1823 – Appeal Request Change – This is the form that is used if you would like to add a step to the appeal process. For example, if you only requested an informal meeting when you originally filed your request, this form can be used to later on add the mediation or hearing. 

DS 1824 Final Reconsideration Request – After the hearing, you or the regional center may request “reconsideration,” which is a way to correct a clerical error, or mistake of fact or law in a hearing decision. This form must be filed within 15 days from the date you received the hearing decision.

Sample good faith belief letter – a good faith belief letter is used by the regional center when there is no other written document (for example, an IPP) that explains the services you have agreed to. The good faith belief letter must be sent to you by the regional center within 5 business days of the day when the regional center believes you agreed.
